Our non-insurance patients have the option of signing up for our in-house dental benefits plan, bringing an affordable option to those without insurance. We also offer a dental savings account where you can select a monthly contribution to deposit automatically into your account for future dental treatment needs.
For our patients without insurance, coverage includes:

YEARLY MEMBERSHIP DUES
Single Adult: $275 per member (Over $200 saving on preventative treatment alone)
Additional Periodontal Plan: $175 additional per member
TERMS AND LIMITATIONS OF THE PLAN
- This is a Dental Discount Plan and is NOT dental insurance. It cannot be combined with any other Dental Insurance.
- It is good only for Dr Zaruhi Shahabazyan Dentistry. Therefore, if you are referred to a specialist, they will NOT offer this discount.
- Treatment for dental injuries covered by workmen's comp, disability insurance, lawsuit, or outside medical care are not covered under this plan.
- This plan is non-transferrable, family members cannot be substituted in for another family member.
- It is NON-Refundable. No refunds give if patient chooses not to use their dental plan.
- Rates are subject to change annually.
- Payments for services are due at time of service. If you choose to extend your payment for treatment using third party financing (CareCredit, ICare, ect.) the plan discount will be reduced by 10%.
- This offer cannot be combined with any other offers.
- For orthodontic treatment, participant must remain a plan participant the entire duration of orthodontic treatment.
- Membership fees are due the day of joining program.
PROGRAM DISCLAIMER
The fees outlined in the Dental Savings plan option or other established Fees by Dr. Zaruhi Shahbazyan Dentistry are non-refundable. All plan fees are due at the time of enrollment. An additional fee may be charged for any missed, cancelled or broken appointment without 48 hours prior notice. This plan is non-transferable by the patient. This plan cannot be combined with any other insurance, dental plan, coupon or discount. Failure to comply with the terms of the plan may result n termination of the plan and forfeiture of any fee paid by the patient or other third-party. Dr. Zaruhi Shahbazyan Dentistry reserves the right to refuse treatment and/or terminate the patient's participation in this plan with 30 days written notice. If the patient's account becomes delinquent or patient is non-compliant. This plan may be modified, amended or cancelled at any time with 30 days prior written notice and may be subject to other terms and conditions. Plan participants are responsible for scheduling their periodic treatments and services not utilized at the end of each year's membership period are not carried over to the following year. Also, discounts apply only to services rendered by Dr. Zaruhi Shahbazyan Dentistry and does not include treatment for services provided elsewhere even if you are referred to a specialist by Dr. Zaruhi Shahbazyan Dentistry. In addition, when third-party financing is used, the plan discount will be reduced by 10% to compensate for merchant fees.
